Mastel was the first company to introduce pre-set diamond scalpels in the 1980’s. Pre-set diamonds are all within the domain of ‘controlled depth’ blade assemblies dedicated to perform only one important task for the eye surgeon, as opposed to our micrometer or step diamond handles.

Our original John Groover was the first to be designed and produced in the late 1980’s with surgeon Maurice John, MD from Jeffersonville, Indiana. It was a simple yet bright idea to make a 300 micron groover for cataract surgery where a multi plane approach was to be utilized. This also doubles as a very good depth for scleral grooves used in glaucoma procedures at the same time. These come in depths of 250, 300 and 350 microns and feature an inexpensive blade for these less demanding wounds.
The Nichamin Classic 600 is for Limbal Relaxing Incisions using the Nichamin Empirical Nomogram for astigmatism correction generally during the time of cataract surgery. This is a far more demanding incision because the refractive results hinge upon being deep in the cornea, so this requires a premium diamond design. There are thousands of these in service around the world today. At the same time, the need for outcomes have advanced and thought should be given to an adjustable diamond today to ensure 90% depth on corneas that vary widely in peripheral thickness as measured by ultrasound.
The highly versatile Fine Triamond is the ideal diamond to pair with this classic design. The highly finished curvilinear footplate provides the ideal applanation for a preset depth AK. The diamond’s truncated tip creates the perfect incision, while the slightly angled edges can cut in either direction.
The PK Zip diamond was born out of the need for advancing the deep flap in viscocanalostomy and/or canaloplasty, but we found that it did not guarantee against perforation of the Descemet’s window being exposed in these very demanding surgeries. Ultimately, we found that the footplate on the bottom with an exposed vertical presentation of a single edge diamond 1mm in height works brilliantly for undermining safely the critical dissection of a penetrating keratoplasty. This replaces the need for scissors in corneal transplant to dissect the remaining stroma and Descemet’s after perforation with corneal trephination and minimizes or eliminates the ‘tags’ associated with scissors dissection.
The President Series handle features a double threaded mechanism for robust, simple actuation. The mechanism locks securely to eliminate free play of the stylus. The titanium construction and elimination of springs makes them virtually indestructible in clinical service. The President serves as the platform for phaco keratomes, paracentesis blades, glaucoma blades, groovers and any freehand application. The diamond is extended straight or at an angle for maximum clearance and accessibility.

Histological and ultrastructural study of corneal tunnel incisions using diamond and steel keratomes
“The corneal incisions made with the dissecting, stiletto-like keratomes (Mastel Fine Triamond) were distinct from the other incisions, having a smoother stromal surface and much less disruption of stromal tissue. The dissecting incision using the stiletto-like diamond keratomes was ideal with respect to the degree of disruption of stromal tissue.”
